Assessment forms

When taking part in STrAWB, you are asked to complete some short forms at the beginning of the study and again 12 months later about the child that you look after or teach. You’ll already have received a detailed email about this, with instructions on which forms to complete, how to complete them and online links (including links to the children’s forms for carers).

Please read the email carefully and complete each form. We ask that they are done within one week. You can complete paper versions of the forms instead if you prefer. Please contact us to request a pack by post: strawb@education.ox.ac.uk.

We have three resources to help with completing the forms:

• A short getting started video on completing our forms online – includes instructions on how to access and complete them using our easy-to-use Qualtrics system.
• A Carer’s Guide and our NEW! Teacher's Guide to the forms – includes some general tips on how to complete the forms as easily as possible, with some ideas for carers on helping the children to complete theirs.

We really recommend viewing these before you make a start. The forms were all designed before the Covid pandemic and we don’t have scope to change the questions, which means that some of them might need a bit of thinking through to answer. The Guides include specific further details about each form, which may be helpful in answering any queries.
