MyView Forms

All forms should be returned to the HR Systems team

  1. Adding staff to MyView

    Please use this form to notify HR of your direct reports that are not showing on your MyView.

    Please note that if a formal request has already been submitted to HR for someone to leave or join your team this may still be in process and may not have been actioned yet. All joiners and leavers should be actioned by the middle of the month that the person has joined or left your department.

    Only use this form if you believe the above action will not rectify the situation.

  2. Removing staff from MyView

    Please use this form to notify HR of direct reports that are showing on your MyView but are not in your team.  If you are aware of the manager they should report please include this on the form.

    Please note that if a formal request has already been submitted to HR for someone to leave or join your team this may still be in process and may not have been actioned yet. All joiners and leavers should be actioned by the middle of the month that the person has joined or left your department.

    Only use this form if you believe the above action will not rectify the situation.

  3. Notification of part time, compressed hours and shift working patterns

    Please use the My Working Pattern form on MyView, under My Forms, to notify HR of your working pattern*.  This will assist you when booking holiday but is not mandatory.

    You do not need to use this form if you are full time and work regular hours and also if you are on a contract that incorporates your holiday pay.

    Full time staff who are working compressed hours or shift work - You must use this form to notify HR that you do not work a usual 5 day week, regardless of whether you also want to submit a working pattern.  Your holiday entitlement will be amended and you will be required to book the hours you would have worked on a bank holiday or minimum service day.

    If a working pattern is not submitted, MyView will spread your weekly hours over 5 days and you will be prompted to confirm your hours in MyView each time you book holiday.  This provides flexibility for those staff that do not work a regular weekly pattern. 

    HR will endeavour to update your working pattern as soon as possible but if you need to book holiday in the meantime you can overtype the working pattern in MyView to match your weekly hours.
